KOTA BHARU: The Department of Occupational Safety and Health (DOSH) has confirmed that unsafe work procedures led to the tragic death of a mechanic assistant at a quarry in Kelantan. The victim was crushed while conducting maintenance work, highlighting serious lapses in workplace safety.

According to DOSH, initial investigations revealed that the accident occurred due to a lack of proper safety protocols and supervision. The worker was reportedly performing repairs on heavy machinery when a structural failure caused the equipment to collapse, fatally trapping him.

DOSH Director-General Ir. Mohd Anuar Embi stated that an immediate stop-work order has been issued to the quarry operator while investigations are underway. “Employers must strictly adhere to safety guidelines, especially when dealing with high-risk machinery,” he emphasized. Legal action will be taken against those found responsible for negligence.

The incident serves as a stark reminder of the importance of workplace safety compliance. DOSH urges all employers to conduct regular risk assessments and ensure workers are adequately trained to prevent similar tragedies in the future.
